During recovery and rehabilitation, it’s important to keep your body and mind active. Between the activities organized by our team and the activities provided by our partnered organizations and the community of New Castle, Kentucky, there is something engaging for New Castle Healthcare residents to do seven days a week.

At New Castle Nursing & Rehab and Twin Oaks Assisted Living, our social services team supports our residents and their families as they transition to our facilities. We have a dedicated social worker team that will provide guidance, support, and advocacy for our residents.
Resident Advocacy
Our social services team supports resident rights, dignity, and personal choice. We advocate for residents to help ensure preferences are respected, concerns are addressed, and each resident feels heard and supported.
Legal Assistance
We help residents and families navigate planning-related matters such as advance directives, power of attorney, and access to community legal resources when appropriate.
Mental Health Services
Emotional wellbeing is an important part of health. Our social worker team can provide support, counseling resources, and referrals to help address adjustment challenges, anxiety, depression, and other concerns.
Discharge Planning & Transitions
Our social workers help residents and families prepare for safe transitions following a stay, coordinating services, resources, and individualized discharge planning to support continued care.
